Privacy & HIPAA Notice

1. Our commitment

Oasis Child and Family Services LLC ("Oasis," "we," "us") is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. This notice applies to all records of care created or maintained by Oasis.

2. Notice of Privacy Practices (HIPAA)

This notice describes how medical and behavioral health information about your child may be used and disclosed, and how you can get access to this information. Please review it carefully. "Protected Health Information" (PHI) is information that identifies your child and relates to their health, care, or payment for care.

How we may use and disclose PHI

• For treatment — to provide, coordinate, and manage your child's care among our clinicians and other providers involved in their treatment.
• For payment — to bill and obtain payment from health plans (for example, Minnesota Health Care Programs or private insurance) for services provided.
• For health care operations — for quality improvement, training, supervision, and the administrative activities needed to run our practice.
• As required or permitted by law — for example, public health activities, reports of suspected abuse or neglect, health oversight, court orders, or to prevent a serious threat to health or safety.

Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

3. Your rights

You have the right to:

• Access and copies — inspect and obtain a copy of your child's health and billing records.
• Amend — request a correction to information you believe is incorrect or incomplete.
• An accounting of disclosures — request a list of certain disclosures we have made.
• Request restrictions — ask us to limit how we use or share certain information.
• Confidential communications — ask us to contact you in a specific way or at a specific location.
• A paper copy — receive a paper copy of this notice at any time, even if you agreed to receive it electronically.
• Be notified — be told if a breach occurs that may have compromised the privacy of your information.

4. Our responsibilities

• We are required by law to maintain the privacy and security of your PHI.
• We will let you know promptly if a breach occurs that may have compromised your information.
• We will not use or share your information other than as described here unless you tell us we can in writing.

5. Website privacy

When you use our online referral form, we collect the information you provide (such as contact details, information about the child, and any documents you choose to attach) so our team can follow up about services. This information is transmitted securely and stored within our Microsoft 365 environment. We do not sell your information. We use industry-standard safeguards to protect data in transit and at rest. Please only upload documents you are authorized to share.

Our website does not provide medical advice and does not knowingly collect information from children. If you contact us by email, please remember that standard email is not always a secure way to send sensitive health information — when in doubt, call us.

6. Phone calls and text messages

We may contact you by phone or text message (SMS) about your child's care — for example, to schedule or confirm appointments, return your inquiry, send reminders, or coordinate services.

• By giving us your phone number (including on our referral form), you agree that Oasis may call and text you at that number, including by automated means. Message and data rates may apply, and message frequency may vary.
• We send only the minimum information necessary by text. Standard text messages and voicemail are not a fully secure way to communicate, and there is some risk others could see them. If you would prefer we not leave details by voicemail or text, just tell us.
• You may opt out of text messages at any time by replying STOP, or ask us to change how we contact you (see Your rights — confidential communications). Opting out will not affect your child's care.
• We will never sell your phone number or use it for marketing unrelated to your child's care.

7. Changes to this notice

We may change this notice and the change will apply to all information we hold. The current notice will always be posted on this page with its effective date.
